The purpose of ASAP’s peer review effort is to strengthen treatment and prevention services throughout New York State. We try to accomplish this goal by including an emphasis on exemplary practices and quality improvement. Each program that volunteers to host a peer review will have an opportunity to showcase one or more specific program innovations that could be nominated, by the reviewer who visits your program, as a Quality Improvement Practice. If selected, your innovative practice will be included in the Independent Peer Review Final Report, your program will receive recognition for the practice, and it will be highlighted at the next ASAP annual conference.
Participation in our peer review process will provide benefits to your agency and staff as well as the field, including the opportunity to:
• gain or share new information, resources and ideas that will help to strengthen services
• promote exemplary practices used in your program or to learn of exemplary practices that you might implement
• gain visibility for the good work being done by your program
• earn re-credentialing credit for Credentialed Alcoholism and Substance Abuse Counselors (CASACS), Credentialed Prevention Professionals (CPP) and Credentialed Prevention Specialists (CPS)
• help your program and NYS meet requirements outlined in state and federal chemical dependence regulations.

All forms below are in MS Word format and or Adobe Acrobat. If you need a different type, contact ASAP. Click the appropriate links and save the file to your computer. You may then open and print the forms are needed. Click here if you do not have Adobe Acrobat
Instructions to download forms
- Right mouse click on the form you want.
- A menu will pop up, click on "Save Target As...".
- Save the file in a folder on your computer you will remember i.e. My Documents.
- Open the folder in your computer and click on the document.
The Independent Peer Review electronic forms (Treatment and Prevention) have been developed using Microsoft Work form tools. In order to complete the tool you only need to fill out the gray areas on the form (indicated by a checkbox or “Click here…). The form is protected so only the gray areas can be changed.
For check boxes simply place your curser on the box (the curser will change to an arrow) and click to check it, or uncheck it.
For “Click here…” areas click on the area and begin typing your response.
