2004 Independent Peer Review Forms
All forms below are in MS Word format and or Adobe Acrobat. If you need a different type, contact ASAP. Click the appropriate links and save the file to your computer. You may then open and print the forms are needed.   Click here if you do not have Adobe Acrobat

Instructions to download forms
1. Right mouse click on the form you want.
2. A menu will pop up, click on "Save Target As...".
3. Save the file in a folder on your computer you will remember i.e. My Documents.
4. Open the folder in your computer and click on the document.

Using the Electronic Independent Peer Review Forms
The Independent Peer Review electronic forms (Treatment and Prevention) have been developed using Microsoft Work form tools. In order to complete the tool you only need to fill out the gray areas on the form (indicated by a checkbox or “Click here…). The form is protected so only the gray areas can be changed.

For check boxes simply place your curser on the box (the curser will change to an arrow) and click to check it, or uncheck it.

For “Click here…” areas click on the area and begin typing your response.
